Abuse Service Search by this category Provision of counselling and treatment services and support to women who have experienced an abusive act or who are in an abusive situation.
Assertive Community Treatment Team Search by this category Multidisciplinary treatment teams providing assertive outreach, individualized treatment, ongoing and continuous services, linkages and include a monitoring and evaluation component.
Alternative Business Search by this category Provision of consumer operated businesses that offer full/part time employment at market rate or higher. They offer combination of job development, job placement and supported education within the self-help context. They may also offer self-employment opportunities for consumers to earn income through independent contract work. Support and accommodation are provided on site to consumer employees.
Case Management Search by this category Case management for a person with a serious mental illness (SMI). This type of services excludes counselling, psychotherapy or other clinical treatment interventions. Case Managers provide at a minimum the following functions: individualized assessment and planning, service co-ordination (linking service recipients with services and supports), monitoring and evaluation of services provided to recipients, and systems advocacy/resource co-ordination. This type of services also includes Community Treatment Order (CTO) co-ordination
Clubhouse Search by this category Provides multi-service psychosocial rehabilitation function to people with a serious mental illness. Based on the psychosocial rehabilitation principles, services for service recipients or members include: assistance to service recipient to further their vocational and educational goals, assistance to service recipient to secure housing, social and recreational programs, excludes drop-in centres that are to be reported using consumer / survivor functional centres.
Community Development Search by this category Provision of information for members of the community regarding community health services and referral to these agencies. Report ONLY if you are specifically funded to provide this service. (e.g., those agencies in North region that are funded to provide supports and services to First Nations communities.)
Community Service Information and Referral Search by this category Information and referral service that take place over the telephone or when the individual inquires in person and no demographic data is recorded. Includes: MH and addictions workers who are involved in only providing information and referral services to service service recipient (e.g., DART), MH and addictions entry services, service recipient who are given comprehensive assessment and referred on to other services, Admitted service recipient who may be referred to other community services in the course of their service plan.
Counselling and Treatment Search by this category Provision of counselling, psychotherapy and other treatment services to the SMI in the community. This type of service also includes tele-psychiatry.
Crisis Intervention Search by this category Pertaining to the response to urgent individual psychological and/or medical needs of service recipient with SMI.
Diversion and Court Support Search by this category Court Support: provided in the courts to service recipient and their families to assist the judiciary, service recipient, and their families with the legal process, to link service recipient to services, and to provide service/support to service recipient.
Early Intervention Search by this category Provision of specialized treatment and support services to service recipient experiencing a first psychotic episode and their families.
Family Initiative Search by this category Pertaining to family groups participating in planning and evaluation of care delivery, as well as provision of services such as self-help, peer support, education, etc.
Health Promotion and Education - General Awareness Search by this category Provision of services pertaining to promoting health, and education the community, including the public, professionals, and other sectors which impact on the health of individuals and populations towards maintaining / improving health statistics related to mental health.
Inpatient Search by this category Pertains to mental health programming provided within a hospital setting.
Home for Special Care Search by this category Pertaining to the provision of long-term residential care to individuals discharged from psychiatric hospitals and / or the SMI who require 24-hour supervision and assistance with activities of daily living.
Peer/Self-help Initiative Search by this category Provision of a range of consumer directed and consumer driven initiatives including self-help initiatives, peer support, drop-in centres and public and provider education about self-help.
Primary Day/Night Care Search by this category Pertaining to the free-standing or attached units where treatment, counselling, rehabilitative / social and recreational services are provided for SMI service recipients who attend for three to twelve hours on average.
Social Rehabilitation/Recreation Search by this category Provision and promotion of opportunities for SMI service recipient to develop inter-person, social, and leadership skills, in order to interact fully in their communities as defined by themselves. Social rehabilitation / recreation require any or all of the following service recipient-directed services: assessment, counselling, planning, consultation with other service providers, service co-ordination, advocacy, monitoring and evaluation. Also services include development of linkages with other service providers to maximize opportunities for social rehabilitation to isolated persons.
Vocational / Employment Search by this category Provision of range of employment supports including: job development / creation / employer outreach, skills development / training for job / education, skills training on the job, job search skills / job placement, employment planning / career counselling, supported education, supports to sustaining education / employment, and leadership training.
Short-Term Crisis Support Beds Search by this category Pertaining to the provision of time-limited emergency housing with high-intensity care for individuals with serious mental illness. This includes services such as assessment, monitoring, care/treatment, symptom stabilization, assistance with securing access to case management and long-term housing services.
Support Within Housing Search by this category Provision of housing support within accommodation for individuals with SMI living either in congregate or individual accommodation requiring varying levels of support. Include: Up to 24-hour support to service recipient and their significant others. This includes individualized assessment and planning; hands on assistance with activities of daily living, co-ordination and support, ensuring a stable housing environment, crisis management, facilitating peer and group support and resident input to their housing environment, Support services provided to individuals who are homeless or at a risk of being homeless, Includes services funded under homelessness Phase II initiative, Includes services provided by housing support workers, Includes services such as connecting with landlords, matching people to housing, housing advocacy, and teaching life skills, Report ONLY if you are specifically funded to provide this service.
